Membership Pricing
Credit/Debit Card is required for membership.
First month’s membership fee is due at time of starting membership. Each additional month will be charged on your credit card on file the same day of the month as the day of membership.
Membership can be canceled at any time with a 30 day written notice but will be charged an early cancellation fee equal to the amount of one month’s membership fee.
Membership term is for 6 months (the initial term). The initial term is a six month obligation agreement, after the initial term it will be automatically continue in monthly automatic payment.
Additional areas cannot be added after membership has begun, however package pricing can be offered for additional areas. Spa Cancelation Policy applies to all no shows or late (within 24 hours) cancellation.

Contract Agreement
I hereby authorize Aesthetics MD to draw monthly and debit from my account equal to Membership value. The Membership benefits are not rollover and may not be transferred. This membership agreement has an initial term for 6 month (the initial term). The initial term is a six month obligation agreement, after the initial term it will be automatically continue in monthly automatic payments. Client may cancel this agreement after the first six months by giving a 30 day prior written notice to Aesthetics MD. The account holder agrees not to request or arrange the return of payments made under this agreement. The account holder warrants and guarantees that all persons whose signatures are required to sign on this account have signed the Pre-Authorized Debit Agreement and that the information set out on this page is accurate and complete. The account holder agrees to notify Aesthetics MD LLC in written form : 1832 Hwy 54 W Fayetteville GA 30214, should any of the information in this agreement changes.
